getting an exterm basically (from what i can gather) means that entire team was dead at once and not just a case of you killing all of them, i.e, maybe you kill the last guy just after the 1st guy respawns, in which case you wouldnt get an exterm.

and yeah ive got a double double exterm in my montage sequel. my theory is that if you get a close call medal itll still keep the timer up.. but thats just me.. maybe it is cause theyre all still dead.. idk

Well what I found weird was I only got a double kill, and then killed 1 guy after the double (which I have no idea why it didn't give me the triple) and then I got the exterm. But I knew it would have happened to other people, I just found it funny because it was the first time it's happened to me, ever.
